Attendee registration will open in mid-April
We are seeking presenters experienced in working with populations from diverse communities that face significant health, social, and economic challenges. We’re looking for abstracts for 60-minute oral presentations describing your practical, tested, culturally-appropriate strategies or promising practices that can be implemented in clinical or community settings, and address any of the following:
Advancing health equity and addressing social determinants of health
Addressing the needs of patients with, or at risk, for chronic diseases
Addressing quality of care, care redesign or operational improvements, team-based care, or care coordination and primary care integration (including medical, behavioral, and/or oral health)
Addressing the unique challenges facing safety net providers
Topics of particular interest include:
Current topics in clinical care such as substance use disorder (SUD), trauma-informed care, and adjusting to the new normal post-pandemic
Innovative care delivery models, including the use of telehealth, remote patient monitoring, or collaborative care
Workforce challenges related to recruitment, retention, or resiliency
Projects focused on transformational leadership and organization-wide improvement efforts
Any other topics related to our 2023 theme of Reuniting for Community Health
